Notice of Privacy Practices (NPP)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Uses and Disclosures of Protected Health Information

Pisgah Eye Surgeons may use and disclose your protected health information (PHI) for treatment, payment, and healthcare operations. Examples include coordinating care with other healthcare providers, billing your insurance, and conducting quality improvement activities.

We may also disclose PHI without your authorization when required by law, for public health reporting, health oversight activities, judicial proceedings, law enforcement purposes, or to avert a serious threat to health or safety.

Uses and Disclosures Requiring Authorization

Any use or disclosure of PHI not described in this Notice will be made only with your written authorization. You may revoke an authorization at any time in writing.

Your Rights

You have the right to: - Inspect and obtain a copy of your medical records - Request an amendment to your medical records - Request restrictions on certain uses or disclosures - Request confidential communications - Receive an accounting of disclosures - Obtain a paper copy of this Notice - File a complaint without retaliation

Our Responsibilities

Pisgah Eye Surgeons is required by law to maintain the privacy of your PHI, provide you with this Notice, and follow the terms of the Notice currently in effect. Complaints if you believe your privacy rights have been violated, you may file a complaint with Pisgah Eye Surgeons or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
